Hydrocephalus is one of the most life-altering conditions affecting children in Uganda and one of the most treatable.
The tragedy is that it sits in a narrow gap between what medicine can do and what health systems can deliver.
What is Hydrocephalus?

Hydrocephalus is a condition where fluid builds up inside the brain.
The brain naturally produces cerebrospinal fluid (CSF), which cushions and protects it. Normally, this fluid flows through the brain and spinal cord and is absorbed into the bloodstream. In hydrocephalus, that flow is blocked or disrupted so fluid builds causing the head to swell.
Why Hydrocephalus Is So Damaging?

The damage caused by hydrocephalus is not just physical—it affects a child’s entire development.
As pressure builds inside the brain, it can lead to:
- Damage to brain tissue
- Delayed cognitive development
- Vision problems
- Seizures
- Difficulty with movement and coordination
- In severe cases, life-threatening brain injury
If left untreated, hydrocephalus can be fatal.
Even when it is not fatal, the long-term effects can be profound, affecting a child’s ability to learn, walk, communicate, and live independently.
The earlier the pressure builds, the greater the risk of permanent damage. This is why timely treatment is critical.
Why It Is So Common in Uganda
In countries like Uganda, one of the most common causes of hydrocephalus in children is infection—particularly infections acquired in early infancy.
These infections can cause inflammation that blocks the normal flow of cerebrospinal fluid, triggering the condition.
Limited access to early medical care can also mean that symptoms are not detected quickly enough, allowing the condition to progress before treatment is possible.
How Hydrocephalus Is Treated

The good news is that hydrocephalus is treatable.
The most common treatment is surgery. There are two main approaches:
1. Shunt Surgery
A thin tube called a shunt is inserted into the brain to drain excess fluid into another part of the body, usually the abdomen, where it can be safely absorbed.
This relieves pressure and allows the brain to recover and develop more normally.
2. Endoscopic Third Ventriculostomy (ETV)
In some cases, surgeons create a small opening inside the brain to allow fluid to bypass the blockage and flow more freely.
This procedure requires specialised equipment and surgical expertise.
The Role of Surgical Equipment

Treating hydrocephalus safely depends heavily on having the right tools.
Shunt systems must be available in the correct sizes for children. Neurosurgical instruments must be precise and reliable. In some cases, advanced tools like endoscopes are needed.
Even supporting equipment matters. Safe anaesthesia and airway management—using tools such as a video laryngoscope —ensure the child can undergo surgery safely. Without these, even life-saving procedures carry significant risk.
What Happens Without Treatment
Without surgery, hydrocephalus often progresses.
The pressure inside the brain continues to increase, leading to worsening symptoms and irreversible damage. In many cases, children may not survive.
This is why hydrocephalus is considered a neurosurgical emergency in children.
But in settings where access to surgery is limited, delays are common—not because treatment doesn’t exist, but because resources are lacking.
The Reality: A Treatable Condition, Still Too Often Fatal
Perhaps the most difficult truth about hydrocephalus is that it is treatable.
In well-resourced health systems, children diagnosed early often go on to live full lives after surgery.
In lower-resource settings, the outcome can depend on something as simple as the availability of a shunt, surgical consumables, or access to an operating theatre.
The gap is not knowledge. It is access.
